AIR-STEMI: new evidence for physiology-guided revascularisation

The AIR-STEMI trial was presented by Associate Professor Simone Biscaglia at ESC Congress 2026 and published simultaneously in the New England Journal of Medicine. EUKON highlighted the study in its clinical updates.

The investigator-initiated trial enrolled 1,823 patients with STEMI and multivessel disease across 21 centres in Italy and Pakistan. It compared functional coronary angiography with conventional angiography to guide treatment of non-culprit lesions.

At a median follow-up of 17.9 months, the primary composite outcome occurred in 8.9% versus 13.7% of patients (hazard ratio 0.62; 95% confidence interval 0.47–0.83). There was no significant difference in all-cause mortality.

The findings support further discussion of selective, physiology-guided complete revascularisation. They should be interpreted within the trial’s population and design.
